PERIODIC REPORTS

1. What Periodic reports during the duration of studies should I submit to TSLB?

  1. Registration form – submitted within two weeks at the beginning of the new academic terms;
  2. Report of grades (in all the subjects you enrolled in)  – submitted at the end of the academic terms. 
  3. Advisory Forms - include any plan for travel/s abroad; changes, if any, in field of study, school, home address, civil status, legal guardian, comments which may be relevant to your study, and such reports that may be required from time to time.
  4. Program Audit Certificate- at the end of each academic year, before 30 December of each academic year.


2. What happens if I do not submit the Periodic Reports?

Your Accounts at TSLB will be placed on hold and no payments for tuition or allowances will be processed and disbursed.

 

3. What happens if I submit my periodic reports late?

Your tagging for tuition or processing of allowances will be delayed and TSLB takes no responsibility. Submission after two weeks of a new academic term may result in loss of allowances as the same will be paid for remaining weeks only.

4. What happens if I do not submit my periodic reports for two or more consecutive academic terms?

Your status will be tagged as non-compliance and you will be suspended for the period you have stopped reporting (no more than 2 academic terms).Non- compliance status beyond two (2) academic terms will result in termination of the award and commencement of the recovery action.

 

5. What should I do if I alter my enrolments after submitting the registration form to TSLB?

You must immediately notify TSLB of any changes. Failure to do so may result in suspension/termination.
